The 52-week prepay brings semaglutide to about $139 per four weeks and tirzepatide to about $209. Prepaid plans renew automatically for the same term unless you cancel, so a 52-week plan renews as another 52 weeks.

KERNX also sells a lower-dose "microdosing" option, priced separately by one- and three-month supply. It's cash pay only, with no insurance billing.

The monthly price includes the medication, shipping, the provider consultation, a monthly check-in with your provider and care-team messaging seven days a week, answered within one business day. Provider-ordered lab testing is included in your second month. If more testing is recommended later, KERNX shows the cost before you go ahead, and you can use your own lab.

There are no health coaches. Support comes from the clinicians and care team. KERNX also runs a free four-week virtual Walk Club, open even if you aren't on medication.

KERNX names its clinicians on its site: Dr. Laurie Mercier and Dr. Michael Wasef, plus nurse practitioners Kimberli Hastings, Theresa Vergara and Craig Sorkin. You're matched with one licensed in your state, and you'll know their name before treatment starts. KERNX says it serves all 50 states and is LegitScript certified.

The free assessment takes about five minutes, and a provider usually reviews it within 24 hours. After approval, the pharmacy takes one to two business days and shipping two to three more, so most first orders arrive three to five business days after approval. KERNX doesn't name the compounding pharmacy publicly. It's identified in your treatment details once you're approved.

• You're not charged until approval. KERNX may place a temporary authorization hold during signup, but it collects payment only after a provider approves treatment and you accept the plan.
• Cancel before your renewal date. You can do it from your account or by email.
• Check the dose-change wording. The headline promise is the same price at every dose, but one FAQ answer says the published price "may change" if your dose changes. Ask your care team to confirm your rate in writing.
• The first months cost the same as the last. A flat price means the starting dose costs as much as the top dose.
• No brand-name option. KERNX offers compounded injections only, and compounded drugs aren't FDA-approved.
