Plans don't renew unless you ask
Each plan is a single upfront charge for its length, and nothing renews on its own. If you want automatic refills, you can choose Subscribe & Save. Each renewal then bills the plan price minus a flat $25 credit, so the monthly plan renews at $165 for semaglutide and $214 for tirzepatide.
To cancel Subscribe & Save, email support before the next billing date shown in your confirmation email. It stops future charges and shipments starting with the next cycle.
Who prescribes and who fills it
FormBlends is a platform, not a medical practice, and it doesn't employ the prescribers. You answer a two-minute assessment covering your goals, state, current medications and health history, with no card needed. An independent licensed provider then decides whether treatment fits. The provider can approve you, deny you, ask for more information or labs, or suggest a different medication.
If you're prescribed, an independent state-licensed 503A compounding pharmacy (one that mixes each prescription for a named patient) fills the order. FormBlends doesn't list it on its site, but the pharmacy's name is printed on the label and the shipping paperwork.
FormBlends dispenses in 36 states and territories, which includes Puerto Rico. Your address is checked during the assessment.
Refunds
If the provider doesn't prescribe, the charge goes back to your original payment method, usually within five to ten business days. Once medication has been dispensed, it can't be returned. A prepaid three-, six- or twelve-month plan is a bigger commitment for that reason, so start monthly if you're unsure how you'll tolerate the drug.
