Balanced Hormone Health, based in Iowa, prices compounded semaglutide and tirzepatide by the total milligrams in a vial. It says plainly that it doesn't sell by length of time. How long a vial lasts depends on your dose, so your real monthly cost depends on how fast you climb.

The starter packages are the easiest way in. Semaglutide starts at $197 for a 5 mg vial, which it estimates lasts about 10 weeks from the lowest starting dose. Tirzepatide starts at $499 for a 60 mg vial, estimated at about a 12-week supply. Bigger vials cost less per milligram, but the largest are for refills only, once you're already on a higher weekly dose.

The price per vial stays the same when you reorder. There's no membership, contract or subscription. You buy the next vial when you need it.

For each drug, BHH lists five "pharmacy packages," each made by a different partner pharmacy with a different added ingredient: plain sterile water, B12, B6, B3 or glycine. The newest glycine package has an estimated use-by date of up to five to six months, which BHH describes as a longer shelf life at a lower price per milligram. Use-by dates are estimates and vary by state.

BHH's FAQ names Tailormade Compounding and Hallandale Pharmacy and describes licensed 503A and 503B sterile compounding pharmacies, which mix drugs either for a named patient or in larger batches under FDA registration. Ask which package your prescription comes from and how long the vial should last at your dose. One customer review on BHH's own site says the vials ran out sooner than expected.

If you want the brand-name drug, BHH also writes Zepbound prescriptions through LillyDirect. That costs $139 for the first telehealth visit and $80 for each monthly refill visit, plus whatever Lilly charges for the medication.

You fill out intake forms online, and BHH texts you within 24 hours on weekdays with the next step. Your visit with a nurse practitioner happens by video through a secure link, and it's free. No labs are needed for weight loss treatment, and BHH sets no minimum BMI.

The price includes the practitioner visit, syringes and alcohol pads, online support and free wellness coaching. BHH says medication takes 4 to 10 days to be made and delivered. It serves the contiguous United States except California.

It doesn't take insurance. It does accept HSA and FSA cards, CareCredit, Afterpay and Klarna.

All sales are final. BHH's refund policy says no refund will be issued, and medication can't be returned once it's dispensed. If the pharmacy sends the wrong drug, dose or label, you must tell BHH within 48 hours of delivery and send photos and proof of purchase. Check your order details before it ships, because that's the only point where a change is easy.
