Center for Medical Weight Loss (CMWL) Review: Price, Pros & Cons

The Center for Medical Weight Loss sells compounded semaglutide and tirzepatide on three plans. Each price covers the medication, supplies, clinical care, membership and shipping, and it's the same at every dose. Month-to-month care is $149 for semaglutide or $239 for tirzepatide. Semaglutide comes on the same three plans at lower prices. On the three-month plan, every third month is free for as long as you stay, which is where CMWL's "every 3rd month free" headline comes from.

One timing detail matters for your budget. CMWL bills its three-month supply options every 10 weeks, so the next box arrives before you run out. That is more than five charges per year, rather than four. Its product page says billed every three months instead, so get the actual renewal dates confirmed in writing.

CMWL started 20 years ago as a network of brick-and-mortar weight-loss clinics and now runs its GLP-1 program online. Its chief medical officer is Dr. Kevin Jovanovic. A U.S. board-certified physician in your state reviews your intake, and a U.S.-licensed compounding pharmacy prepares the prescription. CMWL doesn't name the pharmacies; it says its pharmacy partners are LegitScript-certified. Its compounds include B vitamins or glycine.

You need to be 18 or older with a BMI of 26 or higher. The starter doses add a rule: you can't have taken semaglutide or tirzepatide before, or you must not have injected in the last four weeks. Members also get CMWL Reset, an online lifestyle course, and round-the-clock chat with a provider.

Nothing is charged until a physician approves you, and if you aren't approved you're refunded in full. CMWL's FAQ lists three cases for a refund: you're found ineligible after paying, you don't complete what's needed to ship an order and ask within 30 days, or you accept an alternative your provider recommends. If you cancel an order after your intake has gone to a provider, you get the order amount back minus $50, but only if you ask on the same day as your telemedicine visit.

CMWL says you can pause or cancel at any time with no lock-in or penalty. The annual plan is paid up front, so ask how much of it comes back if you stop early.
