Two charges: the membership and the medication
Primary Clinic is a direct primary care practice, and GLP-1 treatment comes as part of a membership. The membership is $89 a month. Its FAQ is clear that this doesn't include the medication, which you pay for separately at what Primary Clinic calls wholesale cost. Those medication prices are only open to active members.
The medication is priced by dose band, so your bill rises as your dose does.

What the $89 covers
The membership is meant to be your whole primary care, not a weight-loss subscription. It includes unlimited virtual visits, direct text access to your provider, same-day care, routine labs drawn near your home, and what the clinic describes as thousands of routine medications. Records, lab results, refills and messaging run through the Charm health app.
If a provider decides a GLP-1 isn't right for you, the primary care membership is still available. That's worth weighing, since you'd be paying $89 a month either way.
How you get started
There's no instant online checkout. You start with a free virtual consult by phone or text, then join the membership, then have labs drawn and an evaluation. Medication starts only when a provider decides it's clinically appropriate, and it ships to your door. Eligibility generally follows a BMI of 30 or more, or 27 or more with a weight-related condition.
The compounded tirzepatide and semaglutide come from licensed 503A pharmacies, which prepare each prescription for a named patient. Primary Clinic doesn't say which pharmacy fills its GLP-1s; its terms name ManifestRx for routine formulary prescriptions. The practice operates as Georgia Primary Care LLC.
Before you sign up
- The membership has a minimum term. Primary Clinic's terms require at least three months. Leave earlier and you owe the rest of the three months as a termination fee. After that it renews month to month, and you need to give 30 days' written notice to end it.
- Check your state. Primary Clinic serves a limited list of states and says more are coming. Confirm yours on the sign-up form before anything else.
- Ask about the top semaglutide band. No price is shown for 2.25 to 2.7 mg.
- Shipping has a cap. The membership covers up to six free mail-order medication shipments a year; more than that carries a small shipping charge.
- Expect a compounded drug. It doesn't offer brand-name Wegovy or Zepbound, and compounded versions are not FDA-approved.
