FormidableMD is a small physician-led practice based in Florida, and it bills the way a doctor's office does. You pay for visits, and you pay for medication separately. The first weight-management consultation is $199, and follow-up visits are $99. There's no monthly membership on top.

How often you'll see a provider
Every new patient starts with a one-on-one video visit. Your provider has already read your questionnaire and goes over your history, orders labs if needed, and writes a plan. Any change to your medication, whether the formulation, frequency or dose, needs a follow-up visit.
New GLP-1 patients have a follow-up every month for the first three months. Once you're stable on a dose, visits drop to every three months for the first year, and you can request refills through the patient portal in between. Budget for those $99 visits along with the medication, especially early on when you're titrating up.
Insurance and brand-name options
FormidableMD doesn't contract with insurance, but it will check your plan's formulary for FDA-approved GLP-1s at your first visit. If your plan covers one, it sends the prescription to the pharmacy you choose, and you can submit a superbill for the visit to your insurer yourself. Compounded drugs aren't covered by insurance. FormidableMD sources them from 503A compounding pharmacies but doesn't name them.
It treats adults 18 and older in Florida, Georgia, New York and New Jersey. Check your state before you book.
Canceling an appointment
Visit fees are charged to your card 48 hours before the appointment. Cancel more than 48 hours ahead and you get a full refund within three business days. Cancel inside that window and FormidableMD keeps a $75 cancellation fee, because providers start reviewing your file then. If your card fails when the fee is due, the appointment can be canceled.
FormidableMD doesn't publish a separate refund policy for medication, so ask what happens to an order once it's been sent to the pharmacy.
