Wegovy for MASH: The Liver Indication, Trial Results and Limits
The short answer
Wegovy injection is approved for adults with noncirrhotic MASH and moderate-to-advanced liver fibrosis, consistent with stages F2 to F3. Approval is accelerated, based on improvements in liver inflammation and scarring. It does not cover every fatty-liver diagnosis, and the MASH indication does not apply to Wegovy tablets or compounded semaglutide.
Section index07
Finding fatty liver on a scan raises a practical question: does the new Wegovy liver indication apply to me? The answer depends on the diagnosis and amount of scarring, not simply the presence of fat or an elevated liver enzyme.
Wegovy's MASH evidence offers a treatment discussion for a defined group. It is not a reason to order a weight-management subscription before the liver findings have been assessed.
Fatty liver, MASH and fibrosis are different findings
MASH stands for metabolic dysfunction-associated steatohepatitis, previously called NASH. It involves liver fat with inflammation and injury. Fibrosis is scarring; cirrhosis is a more advanced stage of liver scarring. These distinctions matter because an imaging report showing fat does not provide the same information as a diagnosis of MASH with a defined fibrosis stage12.
Ask the clinician to explain the words in your report and which finding has actually been established. If an older record says NAFLD or NASH, ask how that terminology relates to the current diagnosis rather than assuming the underlying condition has changed because its name has.
Who is covered by the Wegovy MASH indication?
The current U.S. label covers Wegovy injection for adults with noncirrhotic MASH and moderate-to-advanced fibrosis, consistent with F2 to F3 stages. The indication does not include cirrhosis, and it does not extend to Wegovy tablets3.
A separate obesity indication may be relevant to some people who also have liver fat. That is a different clinical decision from prescribing specifically for MASH. Ask which condition is being treated and what improvement the clinician plans to monitor. A BMI questionnaire alone does not establish the liver diagnosis or fibrosis stage.
The GLP-1 eligibility guide explains how diagnosis, clinical suitability and insurance approval differ. None should be inferred solely from a service accepting your enrollment.
What did the ESSENCE trial show?
ESSENCE is a randomized, placebo-controlled phase 3 study of weekly injectable semaglutide 2.4 mg in people with biopsy-defined MASH and F2 or F3 fibrosis. The published 72-week interim analysis included the first 800 participants4.
MASH resolution without worsening fibrosis occurred in 62.9% of the semaglutide group versus 34.3% on placebo. Improvement in fibrosis without worsening MASH occurred in 36.8% versus 22.4%, respectively4. These were separate biopsy-based outcomes. Do not read either percentage as the share of patients cured of all liver disease.
Both groups received lifestyle intervention. The study was funded by Novo Nordisk, and longer follow-up is important because tissue changes and long-term clinical outcomes are different measures.
What accelerated approval does and does not mean
FDA granted accelerated approval based on improvement in MASH and fibrosis. Its explanation says the continuing trial will assess whether those changes translate into fewer deaths, transplants and other liver-related events1.
The distinction should be part of the treatment conversation. Ask what is already demonstrated, which outcomes remain uncertain and how that affects the expected benefit in your situation. A promising biopsy result should not be advertised as proof that treatment prevents a transplant for everyone who takes it.
Avoid comparing these percentages with an unrelated weight-loss trial as if they measured the same success. Liver histology, kilograms lost and cardiovascular events answer different questions. Our Wegovy heart-health explainer shows why the endpoint and study population matter.
How is the liver assessment done?
NIDDK describes assessment using medical history, examination, blood tests and imaging, with biopsy in selected cases. Routine imaging can identify liver fat; elastography can help assess stiffness related to fibrosis. Blood-based scores such as FIB-4 can contribute to evaluating advanced scarring2.
Ask which results support your diagnosis, whether more evaluation is needed and who interprets it. Do not assume every patient needs a biopsy because a research trial used one. Conversely, a normal-feeling abdomen or lack of symptoms does not establish the absence of significant disease.
Bring previous laboratory and imaging reports. Ask whether the planned follow-up measures inflammation, scarring, general metabolic health or several of these. The GLP-1 laboratory guide explains how to connect a test to a clinical decision rather than purchasing a large panel without a purpose.
Product choice and follow-up still matter
The MASH approval belongs to a specific FDA-approved injection. It cannot simply be reassigned to compounded semaglutide because the ingredient name sounds similar. FDA explains that compounded products do not undergo its premarket review for safety, effectiveness or quality5.
Discuss the medicine alongside other treatment and any previous adverse reactions. Our semaglutide side-effects guide explains common issues to raise. Ask whom to contact if vomiting, poor intake or another symptom disrupts treatment, and how your liver clinician and prescriber will share information. Do not independently change several medicines while waiting for a routine follow-up.
Work out access after confirming the care plan
Ask the insurer about Wegovy injection for the documented MASH diagnosis, the evidence required and any prior-authorization process. A new indication does not guarantee payment. Keep clinical suitability and affordability as separate questions, and request written reasons if coverage is denied.
The Wegovy and Ozempic cost board can help with brand medication budgeting, but it does not determine which product has the relevant indication. Our Ro review and Sesame Care review describe published service and fee arrangements. They do not establish that either service assesses liver fibrosis or manages MASH.
Before joining an online service, ask directly whether it supports the prescribed treatment, coordinates with your liver clinician and provides the required follow-up. If it cannot, arrange an appropriate care pathway rather than trying to make an inexpensive subscription substitute for the assessment you need.
Frequently asked questions
Does fatty liver on an ultrasound mean I qualify for Wegovy for MASH?
No. The MASH indication requires a more specific diagnosis: noncirrhotic MASH with moderate-to-advanced fibrosis, consistent with F2 to F3. Ask the clinician which findings establish the diagnosis and stage.
Is the Wegovy pill approved for MASH?
The June 2026 U.S. prescribing information lists the MASH indication for Wegovy injection, not tablets. Do not assume formulations have identical approved uses.
Has Wegovy been proven to prevent liver transplants?
The accelerated approval was based on improvements in MASH and fibrosis. Longer follow-up is intended to determine whether these changes translate into fewer transplants, deaths and other liver-related events.
