Ozempic Alternatives: Other GLP-1 Shots, Pills, Compounded Options and What Each Costs

The short answer

The closest alternative to Ozempic is Wegovy, the same drug approved for weight loss. Others are tirzepatide (Mounjaro, Zepbound), the Wegovy pill, Foundayo, and compounded semaglutide or tirzepatide. Liraglutide is older and taken daily. Non-GLP-1 drugs like phentermine work differently, and supplements aren't real substitutes.

What is the closest alternative to Ozempic?

Wegovy. It's the same drug, semaglutide, from the same company, but it's approved for weight loss, where Ozempic is approved for type 2 diabetes12. Wegovy also goes to higher doses: 2.4 mg weekly, with 7.2 mg as an option, where Ozempic stops at 2 mg. If you want what Ozempic does but for weight loss, Wegovy is the direct swap. Is tirzepatide a better alternative?

For weight loss, on average, yes. Tirzepatide is sold as Mounjaro for diabetes and Zepbound for weight loss. In SURMOUNT-5, the head-to-head weight-loss trial, people without diabetes lost 20.2% of their body weight on tirzepatide versus 13.7% on semaglutide over 72 weeks3. Is there a pill alternative?

Yes, two for weight loss. The Wegovy pill is oral semaglutide, taken every morning on an empty stomach. It averaged 13.6% weight loss at 64 weeks in OASIS 44. Foundayo (orforglipron) is a different GLP-1 pill you can take with or without food. Its top dose averaged 11.2% at 72 weeks in ATTAIN-15. It's available, but it isn't an FDA-approved drug and its legal room has narrowed. Since the semaglutide and tirzepatide shortages ended, large-batch compounding of copies isn't allowed. A licensed pharmacy can still compound for an individual patient when a prescriber documents a medical reason. On April 30, 2026, the FDA also proposed keeping semaglutide, tirzepatide and liraglutide off the list of drugs that large outsourcing facilities can compound from raw ingredients78. The comment period closed July 30, 2026, and as of October 2026 no final decision has been published. It's a proposal, not a ban.

What do the alternatives cost?

The compounded figures are medians of published starting prices across the more than 340 programs we track per drug, and many programs charge more at higher doses. The Wegovy and Ozempic totals include a $199 new-patient price on the first two fills, which runs through December 31, 2026. Zepbound's $449 requires refilling within 45 days. Brand "months" are 28 days for pens and vials, so a full year is 13 fills.

What about older or non-GLP-1 drugs?

Liraglutide (Saxenda for weight loss, Victoza for diabetes) is an older GLP-1, and generic liraglutide has been FDA-approved since December 202412. It's a real option, but it's a daily shot rather than a weekly one.

Are "natural Ozempic" supplements an alternative?

Are there natural alternatives to Ozempic?

No supplement works like Ozempic. Berberine, patches and 'GLP-1 support' products are not FDA-approved drugs and have not shown weight loss close to semaglutide or tirzepatide in trials.
