Phentermine: a sympathomimetic amine, not a GLP-1

Phentermine's own FDA-approved labeling classifies it as a sympathomimetic amine anorectic, in the same pharmacologic family as amphetamine, and its prescribing information states it is indicated "for the management of exogenous obesity"1. The mechanism is releasing norepinephrine in the central nervous system, which suppresses appetite — a completely different biological pathway from the incretin (gut hormone) system GLP-1 drugs work through. Phentermine is also typically labeled and prescribed for short-term use (a matter of weeks), reflecting decades-old trial data and its Schedule IV controlled-substance status, whereas GLP-1s are approved and studied for chronic, ongoing weight management. If you've seen phentermine prescribed alongside topiramate (as Qsymia) or on its own at a telehealth weight-loss clinic, it's being used for the same broad goal as a GLP-1 — less appetite, less food intake — but through stimulant-like pharmacology rather than gut-hormone signaling.

Contrave (naltrexone/bupropion): an opioid antagonist plus an antidepressant

Contrave pairs naltrexone, an opioid receptor antagonist normally used for alcohol and opioid dependence, with bupropion, an aminoketone antidepressant. Its FDA label describes it as indicated, in combination with a reduced-calorie diet and increased physical activity, "to reduce excess body weight and maintain weight reduction long term in adults with obesity or overweight in the presence of at least one weight-related comorbid condition"2 — notably, close to the same population definition Wegovy uses. But the mechanism is entirely different: bupropion is thought to activate hypothalamic neurons involved in appetite regulation, while naltrexone blocks an opioid-receptor feedback loop that would otherwise limit that activation, and the combination is also theorized to affect the brain's reward/craving circuitry around food. None of that touches the GLP-1 receptor or the incretin system. It's a real, FDA-approved chronic weight-management drug — just built on repurposed psychiatric and addiction pharmacology instead of a gut hormone.

Jardiance and Farxiga: SGLT2 inhibitors, a diabetes drug class

Empagliflozin (Jardiance) and dapagliflozin (Farxiga) are both classified on their FDA labels as sodium-glucose cotransporter-2 (SGLT2) inhibitors34 — a kidney-acting drug class with no relationship to the incretin pathway at all. The mechanism: SGLT2 is a protein in the kidney's proximal tubule that normally reabsorbs filtered glucose back into the bloodstream. Blocking it causes that glucose — along with the calories it represents — to be excreted in urine instead of reabsorbed. Both drugs are primarily indicated for type 2 diabetes, and both labels also carry expanded indications to reduce cardiovascular death and hospitalization in adults with heart failure, and to slow progression in adults with chronic kidney disease34 — indications neither drug earned through weight loss. Modest weight loss (typically a few pounds) does show up in their trials, but it's a downstream consequence of urinary calorie loss and mild osmotic diuresis, not an appetite-suppression effect, and neither drug carries an FDA obesity indication the way Wegovy, Zepbound, Contrave, or phentermine do.

What actually makes a drug a GLP-1

The defining feature is receptor-level: a GLP-1 medication's own label describes it as a glucagon-like peptide-1 (GLP-1) receptor agonist5 — it binds and activates the same receptor the naturally occurring incretin hormone GLP-1 does, extending a signal that normally lasts minutes into one that lasts about a week. Phentermine acts on norepinephrine release. Contrave acts on opioid receptors and dopamine/norepinephrine reuptake. Jardiance and Farxiga act on a glucose transporter in the kidney. None of the four touch the GLP-1 receptor at all.
