What does Zepbound cost on LillyDirect at each dose?
These are Lilly's self-pay prices, read in October 2026. A "month" is 28 days: four single-dose vials, or one KwikPen holding four weekly doses1.

The vials and the KwikPen cost the same. The single-dose autoinjector pen is the exception: it's priced for people with insurance, at $25 a month for those whose commercial plan covers it and $499 for those whose plan doesn't2.
What is the refill-window rule?
For the 7.5 mg to 15 mg doses, the $449 price comes from what Lilly calls its Zepbound Journey Program. Lilly's terms say you get the $449 offer automatically on your first purchase at those doses. To keep it, you must complete each refill "within 45 days of the delivery/received date" of your previous fill. If you refill later than that, "purchase offer will not apply," and you pay the regular price: $499 at 7.5 mg and $699 at 10, 12.5 or 15 mg2.
In practice, 45 days is about two and a half weeks of slack after a 28-day supply runs out. A vacation, a missed prescription renewal or a pause to deal with side effects can be enough to lose it. One late refill at 10 mg or above costs $250 more than an on-time one. If you fill the KwikPen at another pharmacy and can't get the price at the counter, Lilly's terms allow a reimbursement claim afterward when you were within the window2.

The vial suits people comfortable drawing a dose with a syringe. The KwikPen is easier to use and works at more pharmacies. It's also the only Zepbound self-pay format covered by the Medicare GLP-1 Bridge, which charges eligible Part D members a $50 copay from July 1, 2026 through December 31, 20275. What does a full year cost?
Here are the first 12 fills at Lilly's prices, assuming every refill lands inside the 45-day window. A Lilly month is 28 days, so a full calendar year is 13 fills, and the last column adds the 13th.

Because every dose from 7.5 mg up costs $449, a year at 15 mg costs the same as a year at 10 mg. The late-refill row shows what the 45-day rule is worth: about $2,250 over 12 fills at the top doses. Lilly gives the $449 price automatically on the first 7.5 mg fill, so the regular prices start with the next late refill2. Zepbound cost lists programs that prescribe brand Zepbound.
Is LillyDirect cheaper than TrumpRx or compounded tirzepatide?
Not cheaper than TrumpRx, because it's the same price: TrumpRx sends Zepbound orders to LillyDirect3. See TrumpRx GLP-1 prices for the other brands.
Compounded tirzepatide usually costs less. As of October 2026, across the more than 340 programs we track with a published compounded tirzepatide price, the middle half charge $179 to $299 a month, with a median of $239. That median works out to $2,868 for 12 months, against $5,188 for 12 fills of Zepbound on the standard schedule. Compounded tirzepatide isn't FDA-approved, though, and its legal footing has narrowed. Do you need insurance or a specific doctor?
You don't need insurance. The vial and KwikPen self-pay prices are for cash-paying patients, and Lilly's terms bar billing them to any insurance plan or counting them toward a deductible2. You do need a prescription for an approved use, from any licensed prescriber: your own doctor or a telehealth clinician who sends it to LillyDirect Pharmacy1. Zepbound is approved for weight loss in adults with obesity, or overweight with at least one weight-related condition, and for moderate to severe sleep apnea in adults with obesity4.
LillyDirect also lists independent telehealth services, with visit fees that don't include the medication. Lilly says it doesn't endorse them1.
